Responsibilities:
Completes initial comprehensive and ongoing assessment of patient and family to determine Hospice needsAdmissions of patient for services and development of the initial patient/family Plan of CareUses health assessment data to determine nursing diagnosisCoordination of services with the patient, family, and hospice interdisciplinary team members Requirements:
Valid Registered Nurse (RN) licenseRN Diploma, Associates or B.
S.
degree in nursing Minimum 1 year of Hospice experienceExcellent observation, problem solving and communication skillsCompassion and integrity About the Company:
